Earn a $20 gift card in 20 minutes and help Florida Blue Medicare better serve your health needs.

It only takes 20 minutes to complete your health risk appraisal online through your My Health Link™ account at floridablue.com/medicare. We ask Medicare members to complete the short survey every year to help us understand your health. Your answers help us tailor programs and services to better meet your health needs. And, you’ll qualify for a $20 gift card through HealthyBlue Rewards when you finish this quick survey. Your appraisal should appear on your rewards dashboard within about five days.

It’s easy to complete the appraisal in My Health Link, which is your Florida Blue Medicare member portal.

Here’s how to complete your health risk appraisal on My Health Link:

  • Log in to your My Health Link member account at floridablue.com/medicare by selecting Log in in the top right corner. Once you log in, you’re in your My Health Link member account.

 

  • Select My Health from the menu bar. Then choose My Surveys from the dropdown menu.

  • From the My Surveys menu, you will be able to select and complete your health risk appraisal. Other surveys available to you will appear here, too.

After you complete your appraisal, you’ll qualify for a $20 gift card through HealthyBlue Rewards. You can access your HealthyBlue Rewards account from My Health Link, too.

Just click on the My Health dropdown menu. Then choose HealthyBlue Rewards, which is under My Surveys in the dropdown menu.

This page takes you to your Healthy Blue Rewards dashboard, where you can redeem your rewards for gift cards to popular retailers, like Walmart, Publix and more.

Never used My Health Link before? It’s easy! Here’s how to get started:

  • Grab your Florida Blue Medicare member ID card
  • Visit floridablue.com/medicare.
  • Click Log in.
  • Select New Member Registration and enter your information (including your member number from your Member ID card) to create an account.

We are open 8 a.m. to 8 p.m. local time, seven days a week from October 1 through March 31, except for Thanksgiving and Christmas.
