Adding an Agent to an Agency

If you are a designated producer (agent) selling for a Florida Blue-appointed corporation and need to be added as an agent to your agency, or you are a Florida Blue-appointed corporation and need to add a designated producer (agent) to your agency, you will need to download, complete and return the following forms to the address printed on the forms. An Adding an Agent to Agency checklist is included with detailed instructions for each of the forms. In addition to the forms listed below, you will also need to send a photocopy of your Health Insurance License and a copy of your agency's E&O Proof of Insurance Coverage certificate
