Medical Policies

We strive to cover procedures, treatments, devices and drugs proven to be safe and effective for a particular disease or condition and continually look at new medical advances and technology to determine for coverage and payment purposes if any is superior to those already in use. We provide our Medical Policies (Medical Coverage Guidelines) to our members, physicians and providers so that you are apprised of the criteria used in determining coverage for payment purposes. Appeals & Grievances

  • Late Enrollment Penalty (LEP) Appeals

    Medicare beneficiaries may incur a late enrollment penalty (LEP) if there is a continuous period of 63 days or more at any time after the end of the individual's Part D initial enrollment period during which the individual was eligible to enroll but was not enrolled in a Medicare Part D plan and was not covered under any creditable prescription drug coverage.
