As a Florida Blue Medicare Advantage member, you can get a personalized health assessment with a nurse practitioner in the comfort of your own home. This visit is no extra cost to you, and you can even earn HealthyBlue Rewards just for completing this visit.

  • One-time annual checkup

    A nurse practitioner will come to your home and complete a focused physical exam. During this exam, they will review your medications with you and answer any health questions you may have. They will share the results with you and your primary care doctor so you can discuss them at your next visit. 

    Remember: It’s still important to get your Annual Wellness Visit with your doctor!
