Care at home with Emcara Health

If you have a complex health issue or are managing multiple health conditions, you may qualify for care programs offered by Emcara Health. Emcara Health offers personalized care management, mental wellbeing, and social services support delivered in the comfort of your home. After creating a customized care plan with you, Emcara Health will coordinate your care with your primary care doctor. Emcara Health will reach out to members eligible for the program directly by phone.

Advanced illness care and planning

A trained clinical specialist can help you lay out your advanced directives, so your care aligns with your personal preferences. You’ll also get hospice and palliative care services if you need them.

meQuilibrium

Support your mental well-being, build resiliency, and manage day-to-day stress in the easy-to use app. Getting started is simple. Look for meQuilibrium in the Find and Get Care section of your member account.

These programs are not available to Florida Blue Medicare members.
