What is a Health Risk Assessment (HRA)?
A Health Risk Assessment is a short survey about your health. When you complete this survey, you'll tell us about your current health and where you might need help feeling better. We’ll use the information you provide to give you recommendations tailored to your health needs. The survey is voluntary and only takes about 20 minutes to complete. It's important to note that your answers do not impact your eligibility, premiums, or claims.
How do I complete the HRA?
If you would like to talk to someone to take the survey, call 1-888-927-5223 between 9 a.m. and 9 p.m. ET. You can also complete the survey 24-hours a day/seven days a week by using our automated system by calling toll-free 1-855-718-1537. The HRA is also available online any time.
